An exciting opportunity exists within our Family Health Support Services department to be part of the newly established Community Care Transition Team.
Following a review of Council’s role in aged and community care, Council has made the difficult decision to transition out of delivering certain Community Care services. These changes will better position Council to meet upcoming Aged Care Reform requirements and ensure a stronger, more connected local care provider network.
This new Transition Team will play a key role in supporting clients and community during the upcoming service transition by providing clear information, answering enquiries and guiding people to the right supports. This is a great opportunity to contribute to an important initiative and help our community feel supported throughout this change.
This is a casual role working between 14-30 hours per week. This role will be required until 30 June 2026.
Position details:
- Days/Hours: Casually across Monday to Friday, 9am–4pm (flexibility available-please provide what days/hours you are available)
- Location: Family Health Support Services Department - 43B Davey Street Frankston
Key responsibilities:
- Responding to client and community enquiries related to aged care reforms
- Assisting clients with questions about the services they access
- Providing accurate information and support over the phone
- Working collaboratively within the Community Care team
Support provided:
- Comprehensive training and resources to build confidence in manage enquires
- Ongoing support from the broader Community Care and Management teams

This position is remunerated at a Band 4A - $47.15 per hour plus superannuation.
